  • PET Access Policy - PET Scans Ontario - English
    Policy Criteria Requests will be evaluated against the following criteria 1 Evidence Some literature supports the use of PET scans under these clinical circumstances There is demonstration of greater sensitivity and specificity than standard anatomical imaging OR identification of disease beyond the extent that is defined by anatomical imaging Note No consideration will be given for an indication where the Ministry has decided not to fund due to lack of evidence unless new evidence is presented that supports the use of PET under these clinical circumstances 2 Diagnostic Alternatives There are inconclusive results from standard diagnostic tests 3 Funding Alternatives The patient must not be eligible for a scan under the Insured services or qualify for inclusion in the PET Registry or a clinical trial 4 Clinical Impact The use of a PET scan may result in avoidance of unnecessary invasive interventions e g exploratory surgery or treatment AND OR The clinical management may change based on the PET scan results The clinical impact and implications on clinical management must be described by the referring physician 5 Assessment Applicable criteria must be met before assessment review Requests will normally be considered on a case by case basis by a three person expert panel that will typically consist of a radiologist a nuclear medicine physician and a disease specialist such as an oncologist If a decision cannot be reached by the panel the request will automatically be sent to an alternate panel for consideration Some requests will be adjudicated by PET Scans Ontario staff on a case by case basis Requests for indications that have been approved by the Ministry of Health and Long Term Care but have not yet been implemented Requests for indications that were originally part of a Ministry funded PET Registry or clinical trial that has closed

    fetoprotein and standard imaging tests are negative or a mass persists after primary treatment for seminoma and curative surgical resection is being considered Colorectal cancer where recurrent disease is suspected on the basis of an elevated and or rising carcinoembryronic antigen CEA level s during follow up after surgical resection but standard imaging tests are negative or equivocal or prior to surgery for liver metastases from colorectal cancer when the procedure is high risk e g multiple staged liver resection or vascular reconstruction or where the patient is at high risk for surgery e g American Society of Anesthesiology ASA score 4 Lymphoma for the evaluation of residual mass es following chemotherapy in a patient with Hodgkin s or non Hodgkin s lymphoma when further potentially curative therapy such as radiation or stem cell transplantation is being considered or for the assessment of response in early stage Hodgkin s lymphoma following two 2 or three 3 cycles of chemotherapy when chemotherapy is being considered as the definitive single modality therapy Esophageal cancer For baseline staging assessment of those patients diagnosed with esophageal cancer being considered for curative therapy and or repeat PET CT scan on completion of pre operative neoadjuvant therapy prior to surgery Head Neck cancer For the evaluation of metastatic squamous cell carcinoma in neck nodes when the primary disease site is unknown after standard radiologic and clinical investigation or for the staging of nasopharyngeal cancer Cardiac Myocardial viability assessment in a patient suitable for a cardiac revascularization procedure or cardiac transplantation with moderate to severe ischemic left ventricular dysfunction left ventricular ejection fraction of 40 or less despite maximal therapy and prior myocardial viability or stress imaging assessment with another modality i e SPECT using thallium MIBI or dobutamine stress echocardiography has been equivocal for viability or
